Overview:
Join the hospitalist team at Chestnut Hill Hospital, part of Temple Health, as an Advanced Practice Provider (APP). This role offers flexibility with scheduling, allowing you to balance your work and personal life while providing exceptional care to hospitalized patients in a community-focused environment.

Responsibilities:

  • Conduct daily rounds, collaborating with physicians and specialists
  • Provide comprehensive care, including assessment, diagnosis, and treatment planning
  • Perform necessary procedures within the scope of practice, with physician support as needed
  • Document patient progress and care in EPIC EMR
  • Communicate effectively with patients and families about care plans
  • Flexible scheduling with 15 shifts per month (12-hour shifts)

Qualifications:

  • Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Pennsylvania
  • Hospitalist or inpatient care experience preferred
  • Strong communication, organizational, and time management skills
  • Knowledge of EPIC EMR is a plus
  • Ability to work independently and as part of a collaborative team

Job Responsibilities

Daily Rounding: Perform rounds on hospitalized patients across multiple specialties and conditions. Review and assess patient history, medical records, and progress notes to ensure high-quality care.
Night Rounding: Provide overnight rounding and care to hospitalized patients, ensuring that patient conditions are monitored, documented, and appropriately managed.
Patient Assessment: Complete comprehensive physical exams, conduct diagnostic tests, and assess the condition of patients, providing critical decision-making and clinical judgment to optimize patient care.
Care Planning and Coordination: Develop and implement patient care plans in collaboration with attending physicians, specialists, and multidisciplinary teams. Monitor patient progress and make necessary adjustments to care plans.
Collaboration: Work closely with physicians, nurses, and allied health professionals to ensure continuity of care and timely patient interventions. Provide support for the entire care team in managing complex cases.
Family and Patient Communication: Discuss diagnosis, treatment options, and care plans with patients and families, ensuring clear and empathetic communication. Provide education to both patients and families regarding discharge planning and post-hospital care.
Documentation: Accurately document all patient interactions, assessments, treatments, and care plans in EPIC EMR to maintain compliance with hospital policies and regulatory requirements.
Quality Assurance and Compliance: Adhere to hospital protocols, safety guidelines, and evidence-based best practices. Participate in quality improvement initiatives to enhance patient outcomes and hospital efficiency.

Qualifications

Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Pennsylvania
