Job Description
A health system in Columbus, OH, is actively seeking a dedicated Nurse Practitioner to join their team. In this role, you will lead the medical aspects of team-based care for individuals with serious mental illness, conducting whole-person assessments while collaborating closely with community primary care and behavioral health providers.
Responsibilities:
- Train and advise Medical Assistants and Peer Recovery Specialists on behavioral, clinical, and social health priorities
- Support care coordination efforts
- Lead care and disease management initiatives, identifying actionable care gaps
- Communicate and coordinate with individuals' care providers, primarily at CMHCs, FQHCs, and hospitals
- Provide direct patient care when unscheduled needs cannot be met by existing community providers
- Perform other duties, as needed
Qualifications:
- OH Nurse Practitioner (NP) and/or Physician Assistant (PA) license
Desired Skills:
- 2+ years of experience as a Nurse Practitioner (NP) and/or Physician Assistant (PA)
- Experience working with behavioral health patients
