Job Description
A health system in Columbus OH, is actively seeking a dedicated Nurse Practitioner to join their team. In this role, you will lead the medical aspects of team-based care for individuals with serious mental illness, conducting whole-person assessments while collaborating closely with community primary care and behavioral health providers.
Responsibilities:
- Train and advise Medical Assistants and Peer Recovery Specialists on behavioral, clinical, and social health priorities.
- Support care coordination.
- Lead care and disease management, addressing care gaps.
- Coordinate with care providers at CMHCs, FQHCs, and hospitals.
- Provide direct patient care for unscheduled needs.
- Perform other duties as needed.
Qualifications:
OH Nurse Practitioner (NP) and/or Physician Assistant (PA) license
Desired Skills:
- 2+ years of experience as a Nurse Practitioner or Physician Assistant
- Experience with behavioral health patients
