Seeking a full-time Physician Assistant or Nurse Practitioner for group in Waterford, NY. This is an exciting opportunity to join an existing outpatient practice with strong patient demand and hospital affiliated practice support.

Group provides routine adult healthcare, including care for chronic medical conditions; preventive health and wellness services; cancer screening; and promotion of a healthy lifestyle. With a long tradition of providing high-quality internal medicine to community, the providers provide primary care in a patient-centered medical home setting. This practice is an NCQA Level 3 Patient-Centered Medical Home (PCMH).

Ideal candidate will be patient centered, and possess a strong commitment to providing high quality healthcare to communities we serve, inclusive of the underserved.

REQUIREMENTS

Applicants should have the following:

• Board Certification
• NYS licensure and DEA required
• Applicants should possess excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented
