Are you a new grad or experienced post-acute care nurse practitioner (NP) or physician assistant (PA) who aims to make a difference? TeamHealth needs someone like you! We are seeking an NP or PA to work in a skilled nursing facility rounding on short-term rehab and long-term care patients. This opportunity includes the flexibility to set your own schedule Monday through Friday, start when you want to, and gives you autonomy in your practice.

Key Responsibilities:
  • Perform primary care in a skilled nursing facility setting for short term and long-term patients
  • Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
  • Order and interpret lab results
  • Prescribe and monitor medications
  • Consult with physicians and coordinate patient care with other healthcare providers
  • Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
  • Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
  • Promote healthy lifestyle changes

Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $80,000 to $150,000 annually with no cap on productivity income potential. This is a full-time role starting at 3.5 days per week and will ramp up to 5 days per week once facility needs and schedules are determined.
