A health services network in New York City is currently seeking an experienced, licensed Nurse Practitioner (NP) to join their staff in Manhattan as a Nurse Practitioner Supervisor. In this role, the Nurse Practitioner Supervisor (NP) will be responsible for working with the Community Physician to coordinate and support the participant's functional, clinical, and psychosocial needs, with an emphasis on independence and wellness.

Responsibilities:

The Nurse Practitioner Supervisor (NP) will:

  • Conduct in person visits to cultivate appropriate lines of communication around care coordination and care delivery for each participant
  • Perform routine quarterly clinical assessments on all assigned participants which reflects an understanding of the complexity and multiple co morbidities of the frail and/or elderly
  • Assess and manage participants with acute change in condition in a timely manner
  • Assess participants upon return from the hospital within twenty-four (24) hours or as soon as practical but no later than 72 hours
  • Review patient's past medical history at least every 6 months
  • formulate a comprehensive and complete diagnostic list of current and past medical conditions using clinical knowledge and judgement and the findings of his/her assessment
  • Review current symptoms, exacerbation of problems that were previously controlled
  • identify active diagnoses and chronic problems or conditions to be used in Care Management and active medical management of treatment and designed interventions
  • Educate participants and their families and staff on such topics as disease processes, medication adherence and self- management promotion
  • Perform other duties, as assigned

Qualifications:

  • 2+ years of Clinical Nursing experience in a Home Care, Care Management, and/or a related field
  • Bachelor's and/or Master's Degree
  • NYS Nurse Practitioner (NP) license
  • Computer savvy
  • Microsoft Office
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
