Part-Time NP/PA Post Acute Care Opening in Arizona Located in Show Low, AZ - Sedona 165m, Phoenix 178m We have an exciting opportunity for a Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Pinetop-Lakeside and Show Low, AZ. As part of our team, you'll provide high-quality, safe, and cost-effective care to patients in post-acute care communities while working closely with an interdisciplinary team. This part-time position offers flexible scheduling (1-3 days per week), no primary night call, and the opportunity to maximize your income with a productivity-based compensation model. You ll round on patients in skilled nursing and rehab facilities, focusing on patient care and facilitating smooth transitions to appropriate levels of care.

Responsibilities Patient Care: Provide comprehensive care in skilled nursing and rehab facilities, including H&P, admission, progress, transfer, discharge, and acute/follow-up visits. Care Transitions: Collaborate with the interdisciplinary team to ensure safe patient transfers to appropriate levels of care upon discharge, including home, independent living centers, assisted living, or memory care. Collaborative Approach: Work with hospice, palliative care, and home care agencies to ensure smooth and safe transitions for patients back home or to other facilities. Interdisciplinary Teamwork: Participate in daily rounds and collaborate with clinical and operational teams to provide the best care possible for patients. Desired Qualifications: Certification: Must be a licensed Nurse Practitioner (NP) or Physician Assistant (PA). Experience: Experience in post-acute care, skilled nursing, rehab, or geriatrics is preferred, though not required.
