About the Role:
  • Provide comprehensive primary care services 3 days per week
  • Collaborative with interdisciplinary teams to optimize patient outcomes
  • Ensure smooth transitions of care with hospice, palliative care, and home care agencies
  • Maintain accurate and timely documentation in our EMR system (Gehrimed)

Qualifications:
  • Graduate degree as a nurse practitioner or physician assistant
  • Active and unrestricted NP or PA license in North Carolina
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and interpersonal abilities
  • Experience in post-acute care preferred, but not required (training available)

Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated range of $54k - $88k annually with no cap on productivity income potential.
