**NP/PA Post Acute Care Opening in New Mexico**
Located in Gallup, NM - Albuquerque 138m, Sedona AZ 213m
We are seeking a passionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality, safe, and cost-effective care in post-acute care communities. As part of an interdisciplinary team, you will be responsible for ensuring the well-being of patients through comprehensive care including assessments, progress notes, admissions, discharges, and more.
This position offers flexibility with scheduling, so you can customize your work hours to achieve a work-life balance that fits your needs. You will work closely with local and regional operations teams, as well as with hospice, palliative care, and home care agencies, to facilitate smooth patient transitions and ensure safe care delivery.
Key Responsibilities:
- Conduct health assessments (H&P), progress notes, and perform admissions, transfers, and discharges as required by CMS.
- Work with the facility to ensure appropriate patient transfers to the correct level of care upon discharge, including home, assisted living, or skilled nursing.
- Collaborate with interdisciplinary teams to provide comprehensive patient care and ensure smooth transitions.
- Participate in case reviews and help coordinate care with hospice and home care agencies.
- Contribute to maintaining high-quality, cost-effective, and safe care of patients in skilled nursing and rehab settings.

Qualifications:
- Must be a licensed Nurse Practitioner (NP) or Physician Assistant (PA).
