Responsibilities:
- Providing comprehensive primary care services to a manageable patient panel of 15 patients per day
- Collaborating with interdisciplinary teams to optimize patient outcomes
- Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
- Maintaining accurate and timely documentation in our EMR system (Gehrimed)

- Active and unrestricted NP or PA license in Connecticut (RI additional is a plus)
- Strong clinical assessment and diagnostic skills

- Experience in post-acute care preferred, but not required (training available)
