- November: 1-4, 26-30
- December: 1-2, 10-16, 24-30
- Coverage: Facility #2
- November: 28-30
- December: 24-30
- Site is flexible as Day, swings or nights for those shifts
- No Call
- Facility will accept multiple providers
- Admissions Required
- 18-20 Patients Per Shift
- Beds in Facility#1 – 486, Facility #2 – 25
- Hospital Stroke Certified
- Board Certified – Required
- Active AZ License – Required
- ACLS, ATLS, BLS Required
- Trauma Level I
- EMR: Cerner
