You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:
  • Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day
  • Collaborating with interdisciplinary teams to optimize patient outcomes
  • Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
  • Maintaining accurate and timely documentation in our EMR system (Gehrimed)

Qualifications:
  • Graduate degree as a PA/NP
  • Active and unrestricted NP or PA license in Washington
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and interpersonal abilities
  • 6+ months experience in primary care; experience in post-acute care preferred, but not required (training available)
Compensation is Fee For Service (FFS). Expected compensation is estimated salary range of $157,500 to $187,000 annually with no cap productivity income potential. Full-time clinicians are eligible for an excellent comprehensive benefits package to include: 401k plan, health, dental, and vision insurance, PTO, long and short-term disability, FSA, group term life insurance and employee assistance program.
