**NP/PA Post Acute Care opening in Tacoma, WA** Seattle 33m * Skilled Nursing Facility primary care rounding * Full-time * Monday-Friday, Flexible daily schedule * Telephone call Monday through Friday 7a-7p with no requirement to return to facility. * Ability to set up daily start time. * Board certification required * Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $145,000 to $160,00 annually with no cap on productivity income potential. * Benefit eligible (health, dental, vision, disability, life insurance, 401k w/match, CME, paid time off). **Responsibilities** * Perform primary care in a skilled nursing facility setting for short-term and long-term patients * Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care * Order and interpret lab results * Prescribe and monitor medications * Consult with physicians and coordinate patient care with other healthcare providers * Educate patients and their caregivers on coping skills to handle chronic and age-related * Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery * Promote healthy lifestyle changes **Qualifications** * 1 or more years of experience in primary care * Post-acute care experience a plus but not required * Active and unrestricted NP/PA license in the state of Washington * Active and unrestricted DEA for the state of Washington Tacoma has been known as the City of Destiny for more than 100 years.
