**Key Responsibilities:**_x000D_

* Perform primary care in a post-acute setting for short term patients_x000D_
* Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care_x000D_
* Order and interpret lab results_x000D_
* Prescribe and monitor medications_x000D_
* Consult with physicians and coordinate patient care with other healthcare practitioners_x000D_
* Educate patients and their caregivers on coping skills to handle chronic and age-related conditions_x000D_
* Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery_x000D_
* Promote healthy lifestyle changes_x000D_

**Requirements:**_x000D_

* A minimum of 6 months experience as a PA/NP in a primary care setting required_x000D_
* Post-acute care experience a plus, but not required_x000D_
* Active and unrestricted Washington NP/PA license required_x000D_
* Active and unrestricted Washington DEA with prescriptive authority required_x000D_
