We are seeking a part-time nurse practitioner (NP) to work in a skilled nursing facility environment rounding on patients 3 to 4 day a week. Key Responsibilities:
- Perform primary care in a skilled nursing facility setting for short term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare providers
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
- Critical thinking skills
- Excellent communication skills
- Ability to collaborate with the healthcare team
- Strong clinical skills, work ethic, and ability to work independently
- Adaptability and flexibility to address the fluctuations that occur during the aging process and with chronic illness
- Empathy, compassion and ability to connect and engage with your patients.
- 2+ years of experience in a post-acute care setting required
- Valid NP license in the state of New Jersey
- Valid DEA w/prescriptive authority in the state of New Jersey
