This role involves coordinating care with the attending provider, collaborating with the primary care physician, and managing patient care at home. Responsibilities include assessing patient conditions, prescribing treatments, supporting care plans, and ensuring patient confidentiality. The position also requires participation in ongoing education, documentation of patient interactions, and occasional on-call duties. Qualifications include a Master of Science in Nursing or Doctor of Nursing Practice, certification as a Nurse Practitioner in Oregon, ACLS/BLS certification, and experience in palliative care and hospice.
