Job Opportunity

SSM Health, in conjunction with the Visiting Nurses Association, is actively seeking a dedicated Advanced Practice Provider (NP or PA) to care for patients in the St. Louis Community as part of our Advanced Illness Management (AIM) program.

Job Details:

  • Community and virtual visits
    • Mileage reimbursement available
  • Hours: Monday-Friday, 8a-5p.
    • NO nights/weekends/holidays or call!
  • Mission-driven organization

Minimum Qualifications:

  • Graduate of a nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program
  • Current PA/NP licensure in the appropriate state
  • Experience in palliative care, hospice, or geriatrics highly preferred
