Seeking a patient-centered and empathetic Advance Practice Provider to join our palliative care team. This individual will conduct initial, semi-annual, and unscheduled assessments and reassessments; perform additional physical assessments of participants as needed; participate in the development and implementation of participant plans of care as a member of the interdisciplinary team. Position Details/Highlights: Full-time hospital employed position Will function as a member of the Interdisciplinary Team for collaborative decision making for the participants plan of care.
