Patient Care:

• MD or DO Degree, AND
• Successful completion of an ACGME-accredited residency program, AND
• A passion for providing clinically excellent care
• Successful completion of a Hospice and Palliative Medicine or Geriatric Medicine fellowship, OR
• Current Board Certification in Palliative Care, OR
• Significant experience (minimum 2 years) in a focused Palliative Care program
