  • Work Details:
    • Support staff: Support staff: 1 NP, 1-2 pharmacists, patient coordinator(scheduling)
    • Patients per day: 8-11, blend of video and in person, pharmacist does most of the telephone visits
    • Patient population/age: Mostly elderly population, but can range from 18+. Referred patients from cardiology, transplant, renal services
    • EMR: EPIC
  • Practice Setting: Clinic - Outpatient
  • Credentialing Timeframe: 60 – 90 days

Job Responsibilities

• The Anticoagulation Clinic APP is responsible for the care of patients in the Anticoagulation Clinic. In collaboration with clinical pharmacists, the APP supervises anticoagulation therapy for ambulatory patients through ongoing clinical assessment, ordering and following up on laboratory testing, and modifying medications as needed to meet anticoagulation goals.
• They collaborate with other disciplines or departments to provide care coordination involving anticoagulation management, including perioperative departments, home health, and other procedural areas.

Qualifications

Skills required:
• MUST have long term anticoagulation medication management
• Must have 2 years APP experience
• Minimum of 6mo – 1 year of experience managing patients on anticoagulation therapies (long term anticoagulation, not hospital anticoagulation experience, must be ambulatory/outpatient clinic experience)
Requirements:
• License required: Yes, CA
• DEA: yes, CA with Schedule 2
• Epic exp strongly preferred

Working Hours

• Shift Details: 4 x 8 (7a-3p)

• APP responsibilities include managing meds, checking labs, titrating meds, a lot of in basket through Epic, and a lot of telehealth visits
