A health services network in Georgia is currently seeking a licensed Nurse Practitioner to join their growing team. In this field-based role, the NP will be responsible for conducting patient visits in the home and working closely with providers to deliver high-quality care.
About the Opportunity:
- Schedule: 4 days in the field 8:30am-5pm, 1 day remote doing admin/telehealth
- Location: Dougherty County, GA
Responsibilities:
The Nurse Practitioner will be responsible for:
- Delivering preventive care to members in the comfort of their home
- Completing a comprehensive review and assessment of a member's medical history, physical health, environment and psycho-social status.
- Documenting progress notes, prepare member workups and encounter summaries
- Helping at-risk members transition from the hospital and other facilities to their own home
- Creating personalized care plans in coordination with the member's primary care physician
- Actively closing gaps in care, which may include ordering labs, writing prescriptions with collaboration of the Primary Care Provider or collaborating physician, and coordinating physician follow up
- Performing point of care labs and diagnostic testing, such as A1c, spirometry, and other tests
- Coordinating and communicate with members' interdisciplinary care teams
- Performing other duties, as needed
Qualifications:
- Active ANP, FNP, or GNP license in the state of Georgia
- Active DEA and CDS registration
