Nurse Practitioner or Physician Assistant - Skilled Nursing Facility
SCHEDULE:
- Full-Time
- 5 Days per Week
- 40 Hours per Week
- Monday through Friday
- Flexible Morning Start Time

This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies. They are currently hiring due to growth.
POSITION DESCRIPTION:
The provider is responsible for managing a panel of SNF residents, focusing on treating patients in place in collaboration with a full primary care team.
The provider conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures the care team understands the patients’ goals and needs of care. The provider is supported by a Clinical Manager / Clinical Director, as well as a local Medical Director.
Specific responsibilities include, but are not limited to:
- assess and plan healthcare that is individualized to the patient
- review and interpret medical, surgical, and social histories
- perform screening procedures for patients
- interpret laboratory and other diagnostic data and recommend treatments or actions
- reconcile medications
- be responsible for prescribing or performing various treatment modalities
- work collaboratively with physicians, care coordinators, facility staff, family members, etc.
- perform direct care management support
- educate care teams on patient needs and early identification of changes in condition
- participate in organizational meetings
The practice uses a mobile-based EHR.
The clinical work is autonomous, with one-on-one patient visits and a high level of independence. But it’s also a team-oriented position with meetings, conference calls, etc. with members of the overall care team.
REQUIREMENTS:
- New York NP or PA license
- DEA
- 1+ year advanced practice experience
- 1+ year geriatric experience
- previous experience in SNFs, LTC facilities, etc.
