COMPANY PROFILE:
This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies. They are currently hiring due to growth.
POSITION DESCRIPTION:
The Nurse Practitioner or Physician Assistant (APP) is responsible for managing a panel of SNF residents, focusing on treating patients in place in collaboration with a full primary care team.
The APP conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures the care team understands the patients’ goals and needs of care.
The APP is supported by a Clinical Manager / Director, as well as a Medical Director.
Specific responsibilities include, but are not limited to:
- assess and plan healthcare that is individualized to the patient
- review and interpret medical, surgical, and social histories
- perform screening procedures for patients
- interpret laboratory and other diagnostic data and recommend treatments or actions
- reconcile medications
- be responsible for prescribing or performing various treatment modalities
- work collaboratively with physicians, care coordinators, facility staff, family members, etc.
- perform direct care management support
- educate care teams on patient needs and early identification of changes in condition
- participate in organizational meetings
The group uses a mobile-based EHR.
The APP will work in one (1) SNF location, seeing the same patients and working with the same support team on a daily basis.
REQUIREMENTS:
- active Indiana NP or PA license
- active DEA
- active Medicare credentials
- 1+ year advanced practice experience
- 1+ year geriatric care experience
- previous SNF or related experience
