Job Overview:
We are seeking a dedicated Advanced Practice Provider to join our nonprofit partner in a new and exciting opportunity in their PACE program. The ideal candidate will provide specialized medical care to elderly patients, focusing on promoting healthy aging and managing age-related health conditions.

Duties:
- Conduct comprehensive assessments of geriatric patients' health status
- Develop personalized care plans tailored to each patient's needs and goals
- Manage and coordinate the treatment of medical conditions commonly seen in the elderly population
- Prescribe medications and monitor their effectiveness and side effects
- Collaborate with interdisciplinary teams to ensure holistic care for geriatric patients
-Manages primary care medical practice in PACE Center and in participants’ homes
- Other duties as assigned
Qualifications:
- Graduate degree or post-master certificate required along with completion of accredited Nurse Practitioner Program or graduate of accredited Physician Assistant Program
- Specialization in geriatrics preferred
- Active, unrestricted board certification
- Must possess valid driver’s license and documentation of current automobile insurance.
- Strong communication skills and empathy when interacting with elderly patients and their families
- Bilingual (English/Spanish) highly preferred
