A health services network in New York City is currently seeking a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) to join their Primary Care team in the Bronx. Under general direction, Primary Care NP / PA will be responsible for providing the health status assessment, maintenance, disease prevention and supervision of comprehensive health care.

About the Opportunity:

  • Start Date: ASAP
  • Assignment Length: Ongoing
  • Schedule: 2 days a week
  • Hours: 9am to 5pm
  • Patient Population: Adults with a variety of development disabilities
  • Setting: Clinic

Responsibilities:

  • Obtain complete history of the people we support, recording elements of past health history, family history, review of health habits and environmental data required to diagnosis a condition and render proper treatment
  • Identify and document patient issues appropriately
  • Ensure documentation on the record for the people we support is complete
  • Participate in in-service education for other patient care personnel relevant to program
  • Order and perform necessary tests to ensure proper diagnosis and interpret test results
  • Develop a detailed plan of treatment, therapy and/or rehabilitation and direct patient care team to implement
  • Assist patients and family members with identifying and utilizing resources that may lead to his/her optimum health
  • Prescribe / administer medications, therapeutic diet, immunizations, etc.
  • Explain information to patients and families in an understandable manner
  • Perform other related duties, as requested

Qualifications:

  • 2+ years of experience as a General Practitioner
  • NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license
  • Board Certified
