We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive primary care to a geriatric population through home care, nursing home visits, and outpatient clinic coverage in Amityville, NY. This role offers a manageable patient panel, allowing for high-quality, patient-centered care.
Compensation & Benefits:
- Base Salary: $160,000 - $170,000 plus annual bonus.
- Full benefits package including health insurance, retirement plans, CME allowance, and PTO.
Position Highlights:
- Full-time schedule (4 or 5 days per week) – No nights or weekends.
- Primarily home care/nursing home visits with some outpatient coverage in Amityville.
- Small patient panels for personalized care (4-5 patients per day in the field, 7-10 per day in the clinic).
- Comprehensive primary care with a focus on continuity and proactive management.
Requirements:
- Licensed Nurse Practitioner or Physician Assistant in New York.
- Minimum of 2 years of primary care experience required.
- Experience or strong interest in geriatrics preferred.
- Excellent clinical, communication, and organizational skills.
