• Provide exceptional care to patients in a hybrid in-home and virtual model
• Manage a low-volume patient panel to ensure a focus on quality of care
• Established clinical care team including 2 experienced physicians, 9 APPs, pharmacist, nursing, and social worker support
• Flexible schedule to work around your needs throughout the day - supreme work/life balance offered
• Strong salary range of up to the 70th percentile of MGMA - Anywhere between $120-$150K
