Schedule/Availability Requirements:
- 8-5 Monday thru Friday
- ASAP(as soon as credentialed) for 13 weeks
- Setting: Clinic
- FTE: 1.0
- Duties: Family Medicine clinic visits
- Required Procedures: none
- EMR: Allscripts
- Support Staff: nursing, reception, clinic manager
- Preferred Lodging: we can provide housing
- Air Travel: would prefer local
- Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
- Timeframe: 60 days
Malpractice – Must be included in rates.
Orientation – half the hourly/callback rate, kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours”
Additional Details / Questions Answered by Facility:
- Admissions per day, Patients/day, Types of cases, Disclosures that may need to be made, etc. 6-15 patients per day in clinic - no call or hospitalist duties
Submission Requirements
- Board Certified - REQUIRED (Will consider candidates who are BC in Internal Medicine)
- ND License - REQURIED
- BLS - REQUIRED
- ACLS and PALS - preferred, but not required
- Availability at time of name clear - REQUIRED
- Clean Malpractice - PREFERRED
