Nurse Practitioner Leominster MA
We are looking for a wound care experienced Nurse Practitioner to join our medical group in Central MA area. This position can be part time, with the potential for full time at 20-40 hrs per week depending on case load, locations covered, providing essential wound care services at various assisted living facilities and skilled nursing facilities across Central Massachusetts. Must have a Wound care certification or substantial wound care experience. We cover facilities in the following communities, Athol, Sterling, Worcester, and Westford. Rounds and schedule will vary depending on patient need and census volume.

Responsibilities:
- Deliver comprehensive, patient-centered wound care to residents of assisted living facilities, patients of skilled nursing facilities and home care.
- Perform comprehensive wound assessments and develop treatment plans.
- Implement wound care therapies, monitor patient's response to treatment, and adjust treatments as necessary.
- Communicate effectively with patients, families, and facility staff regarding treatment plans and progress.
- Document all treatments and care provided accurately and thoroughly in electronic medical records.
- Comply with all federal and state regulations regarding wound care treatment and patient privacy.
- Active unrestricted NP (Nurse Practitioner) license in MA
- Must have Wound care certification or substantial wound care experience.
- A minimum of 1 year of clinical experience post-graduation.
- Proficiency in electronic medical records systems.
- Excellent communication, patient care, and team collaboration skills.
- Ability to travel to different assisted living facilities and skilled nursing facilities within Central Massachusetts
- Flexible schedule (20 – 40 hrs/wk depending on case load)
- $60- $75/hour depending on experience
