Are you looking for a unique opportunity where you can effect change and positively impact the continuum of care?

Our client, an established and growth-oriented provider organization, needs an experienced Physician Assistant to provide post-discharge clinical care for a select patient population.

We are seeking a Physician Assistant who is innovative and passionate about identifying ways to improve patient care. As a key member of the Priority Care Team, you will manage patients as they transition from inpatient care to a community or home-based setting.

Collaborating with the PCPs, Population Health, and Case Management, you will provide high-quality, transitional care for more complex patients.

RESPONSIBILITIES:

  • Provision of patient care in outpatient clinic and/or home settings.
  • Provides high quality care to patients as they transition from hospital to home.
  • As an integral member of the Care Coordination Team, works closely with PCPs, Priority Care Physicians, Social Workers, Case Managers, and Pharmacists
  • Attends interdisciplinary meetings to ensure optimal coordination of care.

SCHEDULE:

  • Full Time, Monday – Friday Schedule
  • No nights or weekends

REQUIREMENTS:

  • A valid and unrestricted California PA or NP license
  • Prior experience with CHF/COPD patients a plus
  • Bi-lingual Spanish preferred but not required
  • Minimum 2-3 years prior clinical experience
  • A valid California driver's license, or able to obtain
