We are seeking a compassionate Nurse Practitioner OR Physician Assistant to join our medical practice full time in Rockford, IL. Our ideal Nurse Practitioner/Physician Assistant has tact and skill in patient management, excellent communication skills, and enjoys working as a part of a comprehensive team to offer our patients the best care possible, that they need and deserve! Ideally, we are seeking a provider who has experience with pain management/physical and sports medicine, with the potential to do joint and trigger point injections. This position offers excellent work life balance with no call, no hospital rounds and no weekend hours.

Job Duties:
- Patient care and management- initial medical consults and patient follow up
- Recommend tests and conducts diagnostic testing to determine extent of injury and to determine appropriate treatment.
- Order Diagnostic labs/imaging (NCV/EMG)/allergy testing
- Oversee the patient’s diagnosis and treatments
- Order DME as needed
- Treatments to include joint and trigger point injections (peripheral nerve blocks if trained)
- Documentation - timely and accurate
- Order/refer to in house therapies as needed (Physical Therapy)
- Collaboration with colleagues and staff for comprehensive patient care
- Uphold the company’s mission to provide exceptional patient care and leads in a way that aligns with the company’s goals
- Graduation from an accredited Nurse Practitioner or Physician Assistant program
- Unrestricted NP/PA license to practice in IL
- Experience with pain management/physical and sports medicine, with the potential to do joint and trigger point injections preferred
- Full time (Monday – Friday) Friday office hours end at 1
- No Call or Weekends!
- $120k-$150k per year (depending on experience and training)
