Job Summary:

CarePartners is seeking a nurse practitioner or physician assistant to join our Program of All-inclusive Care for the Elderly (PACE).

Qualified Candidates:
  • Completion of an accredited nurse practitioner or physician assistant program with appropriate certification exams
  • This is an independent role and candidates must have two or more years of experience as a practicing NP or physician assistant treating geriatric, internal medicine or mental health patients
  • Please note that this position does not offer visa support

Position Highlights:
  • Schedules is M-F 8 am to 5 pm with on-call coverage
  • Performs procedures necessary to implement plan of care; consults with interdisciplinary team regarding patient assessment findings
  • Discusses patient plan with patient and family
  • Develops and revises patient education programs and evaluates effectiveness of learning
  • Participates in clinical research and reviews current nursing literature and demonstrates application to clinical practice
