APP Family Medicine Opening in Colebrook, NH -Full Time & Permanent Position. -Experience required. Job Description -Provider must have minimum of 2 years in practice in outpatient primary care, rural health clinic experience preferred. -Able to work autonomously. -Must be Board Certified. -Office location is in Colebrook NH. -Direct patient care time will be split between acute care and established patient panel. -Replacement for provider that is retiring. Primary Care: -Pediatric through Geriatric. -High elderly population. -Experienced in Chronic Disease Management. -EMR is Meditech Expanse, Dragon dictation. -Supported by an RN Team leader and medical assistant.
