NP Family Medicine opening in Halifax, VA
- Located in Halifax, VA - Raleigh-Durham 60m

- Maternal health focus with primary care support for infant, pediatrics, and adults
- Will complete services in Halifax, Danville and Lynchburg
- Minimum of 3 years experience as a licensed provider treating family medicine, pediatrics, and adults with complex issues
- Full-time position with non-traditional hours of noon to 6:00pm or 7:00pm, plus 1-2 Saturdays per month
- Shared call schedule, with RN first call
- Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients health holistically.
- Review quality and outcome metrics monthly with the healthcare team
- Collaborate with local civic and faith-based leaders on community education programs
- Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
