    1. Type: Locum
    2. Position: APP
    3. Role: Clinic
    4. Annual volume: Average 15 pts/day
    5. Any second coverage: No, but there is around 3-5 providers there daily
    6. Shift options: M-F 8am – 4/4:30
    7. EMR: EPIC
    8. Trauma level: Clinic Only, any Emergency would be diverted to Sioux Falls
  2. Candidate Requirements:
    1. State License(s) accepted: SD
    2. DEA/CSR required
    3. Covid/Flu Vaccination Requirements: Required but exemptions available
    4. BC/BE – will site accept non-boarded? Under what conditions: N/A
    5. What boards are accepted? NP or PA
    6. Residents –N
    7. Life Certifications needed: BLS
    8. Will facility accept CALS in lieu of ATLS: Y/N
    9. Procedures logs needed or past amount of experience required and in what timeframe: Logs in the last 12 months
    10. Other qualifications needed: Prefer some experience
