A reputable community health center in Manhattan is seeking a primary care provider to join their team. The ideal candidate will provide high-quality primary care services to patients of all ages, emphasizing preventive care, chronic disease management, and health education.

Key Responsibilities:

  • Conduct comprehensive assessments, including health history, physical examinations, and diagnostic tests.
  • Develop and implement individualized treatment plans based on patient needs and evidence-based guidelines.
  • Manage acute and chronic health conditions, prescribing medications and treatments as appropriate.
  • Collaborate with a multidisciplinary team to coordinate patient care and referrals.
  • Educate patients and families on health promotion, disease prevention, and self-management strategies.

Requirements:

  • Master's Degree in Nursing from an accredited program.
  • Current New York State Nurse Practitioner license and board certification in Family or Adult/Gerontology.
  • Minimum of 2 years of experience in primary care or a related field preferred.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal skills.
