Healthcare United is seeking a qualified Part-Time/PRN Nurse Practitioner/Physician Assistant to join an established Healthcare System in Cleveland/Columbus, Ohio. The Community-Based Provider (CBP) sees patients in their homes, primarily conducting in-home medical assessments and ensuring patients are engaged in active primary care. This position adds to a member’s current care, collaborating with their existing care providers. The role is immensely satisfying, as it involves developing a deep understanding of patients’ needs and engaging a robust care team to address them.
Duties and Responsibilities:
- Conduct comprehensive in-home assessments, including medical history reviews, physical examinations, and medication management
- Perform thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel
- Collaborate with Primary Care Physicians (PCP) on patient education and provide follow-up care
- Provide services in various venues including Home Visits, Skilled Facility Visits, and Telehealth Visits
- Help patients identify barriers to meeting health and life goals and connect them with resources to navigate these barriers
- Educate and coach patients on medication adherence and chronic disease self-management skills
- Monitor members at risk of being lost to care and implement interventions to prevent loss
Requirements
- Licensed Nurse Practitioner or Physician Assistant
Current DEA and ACLS certification - At least 1 year of experience (less than 1 year considered on a case-by-case basis)
- Previous home health experience (Matrix, Signify) is a great fit
- Willing to conduct home health assessments
- Driving record check required
- Must be willing to see patients in their homes
