Our state-of-the-art facilities offer a wide range of medical services, including immediate care for minor injuries and urgent medical situations, alongside virtual visits for follow-up care and consultations for various medical situations. As an Advanced Practice Provider at AllCare Primary & Immediate Care, you will work in a friendly and supportive environment, delivering high-quality medical care to our diverse patient population in Dallas, GA. You will be responsible for seeing bread and butter primary care patients, managing a varied caseload, and providing comprehensive healthcare services.
Locations:
- Dallas, GA
Schedule:
- 3 days a week: Monday to Friday (7:45am-5:45pm)
- Every Other Weekend: Saturday (7:45am - 1:45pm)
Responsibilities:
- Conduct comprehensive assessments of patients' health status.
- Develop and implement individualized care plans for patients.
- Provide primary care services, including diagnosis, treatment, and management of acute and chronic illnesses.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of care.
- Provide patient education on disease prevention, health promotion, and self-care management.
- Participate in discharge planning and transitional care for patients.
- Prescribe medications and monitor psychopharmacology treatment.
Qualifications:
- Hold a valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Georgia.
- Have at least 1 year of employment experience working in a Primary Care/ Urgent Care setting.
- Strong communication skills and the ability to work collaboratively with a multidisciplinary team.
- Master's degree in nursing from an accredited program.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a healthcare team.
- Proficiency in utilizing electronic medical records and other healthcare technology.
