Job Description

The East Bay is a subregion of the San Francisco Bay Area. It is an eclectic and diverse area filled with superb restaurants, shops, theaters, music, and art. The East Bay has easy access west to San Francisco via the Bay Bridge, north to Sacramento, and south to San Jose.

Details:

• Permanent, full-time position

• Work closely with primary care physicians to deliver preventative services to Medicare patients

• Identify diagnoses to be used in care management and active medical management

• Identify urgent situations for proper intervention

• Educate patients on topics related to health conditions

• Conduct Annual Wellness Visits

• Some travel required

• EMR: Epic

Qualifications

• Board Certified or Board Eligible

• Valid California Nurse Practitioner license

• Minimum 7 years of clinical experience required

• At least 3 years of experience as a Nurse Practitioner preferred
