NP Endocrinology opening in Syracuse, NY - Full-time, permanent position A multi-specialty physician group is in search of a Diabetes/Endocrinology Nurse Practitioner who will function as part of a multidisciplinary team of providers and will be given considerable autonomy to provide high-quality comprehensive care in the outpatient setting, focusing on patients with diabetes. About the Position: NP will provide direct care to adult patients with type 1 and type 2 diabetes, weight management, prescribe diabetic medications and titrate insulin, provide patient education on diabetes and prediabetes as well as glucometer and insulin use. Will manage use of insulin pumps, continuous glucose sensors and related diabetes technology. NP will provide direct clinical care for the patient population according to established protocols and utilize evidence-based practice standards NP will perform approved therapeutic or diagnostic procedures based on the patients clinical Status NP will address patient and family concerns and create a therapeutic relationship to provide support. Will provide patient education, counseling, and instruction regarding patient diagnosis, medical treatments, medications, and preventative care, as well as identify patient/family needs for community resources and refers them appropriately.
