· Rural town with a large farming community with both livestock and grain.

· 49 Miles to Hartsfield-Jackson Atlanta International Airport

· Community size: 14,588

· Jasper county remains a rural community, with a large amount of farmland; but it is home to several light industries/manufacturing plants. The County, Public school system, and the hospital are the 3 largest employers in the county.

Start Date: July 2024

Credentialing: 60 days

Schedule: 7-10, 24-hour shifts/month

Call: 24/7 call coverage

· SNF Beds: 55 beds in attached nursing home

o Codes are typically brought to the ED not responded to in the SNF

o No rounding is required on this unit

· Average Daily Census: 4.4 medical floor, 11 ED

· Avg. Monthly Admissions: 7

· Average Monthly Discharges: 1

· Avg LOS: 93 hours IP, 23 days swing bed

Acuity: Respiratory (URIs, asthma, pharyngitis, strep), Viral (flu, Covid), Uncontrolled Diabetes, Uncontrolled HTN, CP, Lacerations, Sprains, Headache, Back pain

Experience + Qualifications

· Minimum 2 years of experience practicing autonomously

· Ability to perform all procedures

· Active GA License or the ability to obtain

Compensation & Benefits

· Independent Contractor (1099)

· Malpractice Insurance Covered by RPG

· Providers have an apartment in a detached building that houses administrative offices and some ancillary departments. The apartment is a new construction (2022) and consists of a small sitting area with TV, Dish Network, computer and PACS station, separate sleeping area with twin size bed, desk, small refrigerator and microwave, and a large bathroom with shower.

o Linens and housekeeping provided on a daily basis.

· Providers are provided 3 full meals a day from the dietary department.
