- Average Patients seen: Carry a patient panel of a minimum of 1500 patients.

Responsibilities: - Provides direct care and education to Health Center patients and ensures continuity and quality of care. - The provider will have a solid understanding of patient needs and will work collaboratively with a multidisciplinary team to co-manage primary care patients. Qualifications: - Must be eligible for or already hold an active Massachusetts license as a Physician Assistant. - Must maintain a current Massachusetts Controlled Substance Registration (MCSR) and DEA registration. - Must apply for and be granted privileges at affiliated hospital(s). - Understanding of ambulatory family practice health care program administration and clinical operations preferred. - Experience with Patient Centered Medical Home model and concepts preferred. - Fluency in Spanish is required; proficiency in Portuguese is a bonus.
