**APP Cardiology Opening in CT** Location: Hartford, CT Full Time **Position Details** Advanced Practitioner Role Scope - Nurse Practitioners (APRN) / Physician Assistants provide cardiology consultations within the Skilled Nursing Facility setting - Assess newly admitted heart failure/post MI patients to the SNF setting and weekly follow up visits - Collaborate with other providers on Cardiac Care with the goal of reducing hospital readmissions - Assist in helping to coordinate discharge and provide referrals to Adv HF therapies Advanced Practitioners Enjoy - Direct support from the Chief of Cardiology to help create a new cardiology consultation program within the Skilled Nursing Facility setting - Autonomy to create your own workflow and schedule - Only travel between sites located closely together - Future leadership opportunities as the new program grows - Call not required - Competitive compensation, matching 401k, tiered medical, dental, and vision plans - Reimbursement for CME, professional society membership, licensure, and malpractice **About Hartford** Hartford, Connecticut, is a historic city established in 1635.
