**Position Details**
Advanced Practitioner Role Scope
- Nurse Practitioners (APRN) / Physician Assistants provide cardiology consultations within the Skilled Nursing Facility setting
- Assess newly admitted heart failure/post MI patients to the SNF setting and weekly follow up visits
- Collaborate with other providers on Cardiac Care with the goal of reducing hospital readmissions
- Assist in helping to coordinate discharge and provide referrals to Adv HF therapies

- Direct support from the Chief of Cardiology to help create a new cardiology consultation program within the Skilled Nursing Facility setting
