We are seeking a Permanent Nurse Practitioner in Troy, NY.
Permanent Nurse Practitioner
Position Type: Full-Time

Shift: Monday-Friday

End Date: Ongoing
Position Details:
• Patient Population: Adult patients with ostomy and wound care needs.
• Setting: Inpatient and outpatient care at Samaritan Hospital
• Responsibilities:
o Conduct assessments of ostomy patients, evaluate wound status, skin integrity, and overall health.
o Develop individualized care plans and collaborate with physicians, nurses, and other healthcare professionals.
o Provide patient education on ostomy care techniques and follow-up care.
o Utilize advanced wound care techniques and maintain accurate patient records.
• Required Procedures: Wound care, ostomy care.
• EMR System: Not specified
• Support Staff: Work closely with Colorectal and Urology surgeons.
• Days Per Month: 20-22 days (based on full-time schedule)
• On-Call/Weekends: No weekends or on-call required.
Submission Requirements:
• Board Certified NP (by time of start) – Required
• NY License (or willing to obtain) – Required
• WOCN Certification – Required
• DEA (or willing to obtain) – Required
• Experience: At least 1 year of NP, preferably in wound care management with ostomy care – Highly Preferred
