Job Summary
This role focuses on assessing, creating, and overseeing treatment plans for residents in senior living communities and those receiving in-home wound care. Wound care providers are responsible for diagnosing and treating different types of wounds, performing lab tests, providing referrals, and carrying out bedside procedures based on their skills and available tools. Working closely with both internal and external healthcare providers is crucial to ensure complete care for patients in both home and facility settings.
Duties
- Perform wound debridement at the patient's location using various methods.
- Manage topical wound care and related products.
- Recommend services like tests and referrals to support healing.
- Review diagnostic results and suggest follow-up care.
- Prescribe medications, DME, or supplies per the supervising physician's agreement.
- Assess Diabetic Foot Ulcer risk following ADA guidelines.
- Educate patients on wound care and update referring physicians.
- Provide guidance on health decision-making and planning.
- Collaborate with staff to address resident needs.
- Conduct quarterly skin assessments.
