Position Overview:

The TCM Advanced Practice Clinician (APC) will focus on providing comprehensive post-discharge care primarily through virtual telemedicine visits mostly, 8 days a month dedicated to in-person clinical care. The role will emphasize managing a wide range of neurological conditions, with a special focus on stroke care and other neurovascular disorders.

Assessment & Management

  • Perform thorough history and physical exams, focusing on neurological assessments, and document findings in the medical record.
  • Order and understanding of diagnostic tests relevant to neurological conditions, including neuroimaging, EEGs, and lab work.
  • Identify and manage risks associated with the care of stroke and other neurologically complex patients, such as secondary stroke prevention, post-stroke spasticity, and seizure management.
  • Collaborate with interdisciplinary teams to triage and manage acute neurological symptoms, ensuring timely intervention and follow-up care.

Diagnosis

  • Work closely with the interdisciplinary healthcare team to diagnose and manage neurological conditions, with a focus on stroke, TIA, and other cerebrovascular disorders.
  • Oversee and interpret diagnostic tests, formulating differential diagnoses for acute and chronic neurological conditions.
  • Diagnose and manage complications associated with stroke and other neurological disorders, such as recurrent stroke, hemorrhagic transformation, and neurogenic dysphagia.

Formulates Plan of Care

  • Develop individualized care plans for patients with stroke and other neurological conditions, incorporating evidence-based practice guidelines for neurorehabilitation, secondary prevention, and chronic disease management.
  • Implement and adjust care plans to address evolving patient needs across the continuum of care, from acute recovery to long-term management.
  • Prescribe appropriate diagnostic strategies and therapeutic interventions, including medications, physical therapy, and neurorehabilitation services.
  • Perform advanced neurological procedures consistent with privileges and validated competencies.

Documentation

  • Document key components of patient care, including detailed neurological assessments, diagnostic results, and individualized care plans. Ensure documentation is timely, meets ambulatory care compliance standards, and captures the complexity and acuity of neurological patients.

Communication and Collaboration

  • Maintain ongoing communication and collaboration with the interdisciplinary neurology care team, including stroke coordinators, rehabilitation specialists, and case managers.
  • Facilitate clear communication with patients, families, and healthcare staff to ensure continuity of care across the inpatient and outpatient continuum, with a focus on stroke and other neurological conditions.

Professional Practice

  • Demonstrate professional practice behaviors, including mentoring and educating graduate students, particularly in stroke and neurological care.
  • Engage in research opportunities and contribute to the analysis and application of evidence-based practices in stroke care and neurorehabilitation.
  • Actively participate in Advance Practice Grand Rounds and neurology-specific meetings, staying current with continuing education and professional development requirements.
  • Master’s degree in nursing or a related field, with certification as an Advanced Practice Clinician.
  • Experience in neurology, with a strong focus on stroke care, neurorehabilitation, and post-acute neurological management.
  • Commitment to interdisciplinary collaboration and continuous professional development in the field of neurology and stroke care
  • National Board Certification as an Nurse Practitioner or Physician Assistant
  • BLS and ACLS certification
