We are seeking a dedicated Nurse Practitioner to join our Rheumatology department in a full-time capacity at our Wethersfield, Connecticut location. Job Responsibilities

Conduct comprehensive evaluations, diagnoses, and treatments for patients with rheumatologic conditions.

Collaborate closely with physicians and the multidisciplinary care team to provide integrated, patient-focused care.

Manage follow-up care plans, monitor patient progress, and adjust treatment plans as necessary.

Educate patients and families on disease management, treatment options, and preventive care.

Maintain accurate and compliant documentation in the EMR system.

Contribute to quality improvement initiatives and patient satisfaction goals.

Qualifications

Valid and unrestricted Connecticut APRN license (or eligibility for licensure).

Certification as a Nurse Practitioner (FNP or Adult-Gerontology preferred).

Current DEA registration or eligibility to obtain one.

New graduates are encouraged to apply; prior rheumatology experience is preferred but not required.

Excellent communication and interpersonal skills for patient education and collaboration.

Ability to work both independently and as part of a multidisciplinary care team.
