Job Responsibilities
Deliver high-quality primary care and conduct routine and acute office visits.
Administer annual wellness visits and health risk assessments.
Coordinate care with specialists and lead population health initiatives.
Educate patients on their health conditions and treatment plans.
Collaborate with a supportive care team in an innovative environment.
Qualifications
Master’s degree in Nursing or NP Certificate.
National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner.
Active state Nurse Practitioner license and DEA license.
Experience in primary care preferred.
