Job Overview - Nurse Practitioner Supervisor (Mobile Program):
Step into a leadership role as a Nurse Practitioner Supervisor for our client's Mobile Program in Bronx, NY, overseeing participant care coordination with PACE Physicians and community primary care providers. This position is dedicated to supporting participants' functional, clinical, and psychosocial needs, fostering independence and wellness.

Compensation: $180,000 - $190,000/year
Location: Bronx, NY
Schedule: Monday to Friday, 8:00 AM to 5:00 PM


Responsibilities as the Nurse Practitioner Supervisor:
  • Clinical Oversight: Supervise Nurse Practitioners and assist with managing clinical staff at assigned nursing homes and PACE sites, ensuring quality participant care.
  • Assessments: Conduct wellness assessments and manage care for high-risk participants, coordinating with primary care providers (PCPs) and interdisciplinary teams (IDT).
  • Coordination: Oversee the development and implementation of individualized care plans, monitor clinical outcomes, and ensure accurate documentation.
  • Participant Care: Provide urgent care services, facilitate same-day access, and conduct home visits as needed.
  • Supervisory: Serve as a liaison between participants, community physicians, and nursing homes while overseeing home care services.
  • On-Call Duties: Participate in the on-call rotation and provide emergency coverage for PACE center primary care providers when necessary.
Qualifications for the Nurse Practitioner Supervisor:
  • Education: Graduate of an NYSED-approved nurse practitioner program or nationally certified.
  • Experience: 2+ years in clinical nursing practice, home care, or care management (frail or elderly populations preferred).
  • Skills: Strong clinical, communication, and organizational skills, with high-level analytical and research abilities.
  • Licensure: Active Nurse Practitioner license in New York required.
  • Certification: DEA Certification and Board Certification from AANP or ANCC are required.
