Compensation: $180,000 - $190,000/year
Location: Bronx, NY
Schedule: Monday to Friday, 8:00 AM to 5:00 PM
Responsibilities as the Nurse Practitioner Supervisor:

Responsibilities as the Nurse Practitioner Supervisor:
- Clinical Oversight: Supervise Nurse Practitioners and assist with managing clinical staff at assigned nursing homes and PACE sites, ensuring quality participant care.
- Assessments: Conduct wellness assessments and manage care for high-risk participants, coordinating with primary care providers (PCPs) and interdisciplinary teams (IDT).
- Coordination: Oversee the development and implementation of individualized care plans, monitor clinical outcomes, and ensure accurate documentation.
- Participant Care: Provide urgent care services, facilitate same-day access, and conduct home visits as needed.
- Supervisory: Serve as a liaison between participants, community physicians, and nursing homes while overseeing home care services.
- On-Call Duties: Participate in the on-call rotation and provide emergency coverage for PACE center primary care providers when necessary.
- Education: Graduate of an NYSED-approved nurse practitioner program or nationally certified.
- Experience: 2+ years in clinical nursing practice, home care, or care management (frail or elderly populations preferred).
- Skills: Strong clinical, communication, and organizational skills, with high-level analytical and research abilities.
- Licensure: Active Nurse Practitioner license in New York required.
- Certification: DEA Certification and Board Certification from AANP or ANCC are required.
