Psychiatric Nurse Practitioner - Assertive Community Treatment (ACT) Program

Location: Upstate New York

Position Overview

We are seeking a dedicated Psychiatric Nurse Practitioner for our Assertive Community Treatment (ACT) program. This full-time, permanent role offers a supportive and collaborative environment to deliver comprehensive psychiatric care to adults with severe and persistent mental illness.

Schedule: Monday to Friday, 8:00 AM - 4:30 PM

Key Responsibilities

• Conduct psychiatric assessments, including diagnosis and mental status exams
• Provide ongoing psychiatric care, including medication monitoring and management
• Collaborate with the ACT Medical Team, including a Psychiatrist and RN
• Explain the role of NP and medications in mental health treatment and recovery
• Manage client crises both on-site and over the phone
• Offer wellness activities such as health monitoring and consultations
• Assist in training staff on treating severe and persistent mental illness

Qualifications

• New York license in hand or in process
• 1-2 years of experience in mental health settings
• Knowledge of evidence-based practices and person-centered care is a plus
• Valid driver’s license and own transportation
